I hereby adopt the animal described above, and release Renee’s Rescues, from all claims for personal injury and/or property damage to myself or others associated with this adoption. The welfare of this animal will be my sole responsibility. I AM NOT ADOPTING THIS ANIMAL FOR ANOTHER PERSON. This animal will not be used for breeding, medical or experimental purposes.

HEALTH RESPONSIBILITY(Required)

I agree to the HEALTH RESPONSIBILITY Policy

After adoption, this animal’s health becomes my full responsibility. I will provide necessary veterinary care upon sickness, disease, or injury. In addition, the dog will be treated by a licensed veterinarian in order to assure and maintain the health of the dog. The dog will receive a rabies vaccination and be licensed in accordance with state and local ordinances. The dog will be tested for heartworm, and a heartworm preventative will be administered as prescribed by a veterinarian.

Adopter understands and agrees to have the dog seen by a licensed veterinarian within 15 days of the date of this contract.

SPAY & NEUTER(Required)

By electronically signing this contract, I understand that all dogs through Renee’s Rescues, Inc. must be spayed or neutered.

If the dog has not been altered as of the date of this contract, I agree to be present and bring my newly adopted dog on the future scheduled time and date provided to me by Renee’s Rescues AND/OR Animal Veterinary Center. Failure to appear on the morning of surgery will result in this contract becoming void and the dog being reclaimed by the rescue.

PREGNANCY(Required)

I agree to the PREGNANCY Policy

By electronically signing this contract, I understand that if my dog is determined to be pregnant prior to the scheduled spay/neuter surgery date, Renee’s Rescues is the legal owner of the puppies and claims all ownership, right and interest in the puppies.

I understand that Renee’s Rescues saves dogs from time-sensitive situations and does not assess for pregnancy unless evident from physical appearance.

CONTROL(Required)

I will keep this animal in a private residence only. The dog will be kept on a leash or in a fenced-in yard when outside.

TRANSFER OF OWNERSHIP(Required)

If I can no longer keep this animal, I MUST contact Renee’s Rescues by telephone or email immediately to arrange for re-adoption. I will never abandon this animal or give him or her to a shelter, organization or another person. I will ensure that this animal is never sold, given up for experimentation, or exchanged in any form. I will abide by the terms of this contract and allow sufficient time to arrange for re-adoption or foster care.

I understand that I may be contacted for further verification of the animal’s welfare and will cooperate by providing whatever information is requested which could include a visit into my home.

RELEASE(Required)

I do not hold Renee’s Rescues responsible for errors in the information provided to me about this animal. Further, I understand and agree that Renee’s Rescues CANNOT GUARANTEE THE HEALTH, TEMPERMANT, and TRAINING OR SUITABILITY of this dog for any particular purpose. The undersigned adopter does hereby release and covenant to hold harmless Renee’s Rescues from any claims, damages, costs or actions incurred as a result of this adoption or caused by the actions of the dog transferred herein. I accept all responsibility and liability for the animal at the date of adoption.

Required Disclosures & Acknowledgements

IF THE DOG YOU’RE ADOPTING IS NOT YET FIXED, YOU MUST ADHERE TO THE FOLLOWING GUIDELINES:

UNDER 6 MONTHS OF AGE: YOU MUST GIVE US A $75 CHECK DEPOSIT, WHICH WILL BE DESTROYED ONCE THE PROCEDURE IS COMPLETE. THE DOG MUST BE SPAYED/NEUTERED BY 6 MONTHS OF AGE. AN APPOINTMENT MUST BE SCHEDULED BY THE TIME THE DOG IS 4 MONTHS, AND YOU MUST NOTIFY US OF THE APPOINTMENT DATE. YOU ARE ALSO RESPONSIBLE FOR SENDING US DOCUMENTATION CONFIRMING THE ANIMAL HAS BEEN FIXED. SHOULD THIS NOT BE DONE AS OUTLINED HERE, YOUR DEPOSIT CHECK WILL BE CASHED AND WILL NOT BE REFUNDED. SEND NOTICE OF APPOINTMENT DATE AND SUBSEQUENT PROOF OF SPAY/NEUTER PROCEDURE TO [email protected]

FOSTER-TO-ADOPT DOGS THAT ARE OVER 6 MONTHS OF AGE: YOU MUST GIVE US A $75 CHECK DEPOSIT, WHICH WILL BE DESTROYED ONCE THE PROCEDURE IS COMPLETE. A SPAY/NEUTER APPOINTMENT MUST BE SCHEDULED WITHIN 14 DAYS OF TAKING THE DOG HOME. YOU MUST NOTIFY US OF THE APPOINTMENT DATE, AND ARE REQUIRED TO SEND US DOCUMENTATION CONFIRMING THE DOG HAS BEEN FIXED. SHOULD THIS NOT BE DONE AS OUTLINED HERE, YOUR DEPOSIT CHECK WILL BE CASHED AND WILL NOT BE REFUNDED.

THERE IS MUCH DEBATE ABOUT WHAT THE APPROPRIATE AGE IS TO HAVE A DOG FIXED. WE DO NOT HAVE A SET STANCE ON THIS, BUT WE ARE REQUIRED TO FOLLOW THE REGULATIONS PUT IN PLACE. DELAWARE CODE STATES THAT ANY ANIMAL ADOPTED FROM A SHELTER OR RESCUE MUST BE SPAYED OR NEUTERED BY 6 MONTHS OF AGE. FAILURE TO DO SO CAN RESULT IN A $250 FINE FOR THE ADOPTER AND A $500 FINE FOR THE RESCUE.
